DHI Hair Transplant: Is It Worth the Extra Cost?
Direct Hair Implantation uses a special Choi pen for more precise results — but is it right for you?
What Is DHI?
Direct Hair Implantation (DHI) is an advanced hair transplant technique that uses a specialised tool called a Choi Implanter Pen. Unlike standard FUE where extraction and implantation are separate steps, DHI combines channel creation and graft placement into a single motion.
The Choi pen is a hollow needle that holds a single graft. The surgeon uses it to simultaneously create a tiny channel and implant the graft at the precise angle, depth, and direction needed. This gives the surgeon exceptional control over the final result.
DHI is particularly popular for:
• Patients who want maximum density
• Hairline work where precise angle control is critical
• Patients who want an unshaven procedure
• Those with existing hair who want to add density without shaving
DHI vs FUE: Key Differences
Extraction: Both DHI and FUE use the same extraction method — individual follicle extraction with a micro-punch. The difference is in the implantation.
Implantation: FUE requires pre-made channels (incisions), then grafts are placed into them. DHI creates the channel and places the graft simultaneously using the Choi pen.
Shaving: FUE typically requires full shaving of the donor area. DHI can be performed without shaving (unshaven DHI), though this takes longer and costs more.
Density: DHI can achieve slightly higher density because the Choi pen allows grafts to be placed closer together without damaging adjacent follicles.
Time: DHI generally takes 1-2 hours longer than FUE for the same number of grafts because each graft is individually loaded into the Choi pen.
Cost: DHI is typically 20-40% more expensive than FUE due to the specialised equipment and longer procedure time.
Results: Both techniques produce excellent results with experienced surgeons. The difference in final outcome is often minimal — surgeon skill matters far more than technique.
DHI Recovery and Results
DHI recovery is similar to FUE, with a few advantages:
• Slightly faster healing due to smaller implantation wounds
• Less bleeding during the procedure
• Potentially higher graft survival rate (some studies suggest 90-95% vs 85-90% for FUE)
• Less time outside the body for each graft (grafts are implanted immediately after extraction)
The growth timeline is identical to FUE: shock loss at 2-4 weeks, new growth at 3-4 months, noticeable results at 6-8 months, and final results at 12-18 months.
DHI Cost for Canadians
DHI in Turkey typically costs $3,000-$7,000 CAD for an all-inclusive package. This compares to $15,000-$30,000 CAD in Canada.
The premium over FUE is justified if you:
• Need precise hairline work
• Want an unshaven procedure
• Want maximum density in a specific area
• Have existing hair you want to preserve
For large area coverage (3,000+ grafts), many surgeons recommend a hybrid approach: DHI for the hairline and FUE for the crown, combining the precision of DHI where it matters most with the efficiency of FUE for larger areas.
